To get started, select one of the applications forms listed below, which will open on your computer as a PDF document.
The first step is to select the proper application form. Several versions of the form are available for download on this page (see below for guidelines on selecting the proper version). Once the form is downloaded, we suggest that you print a copy and fill it out manually. Those with access to Adobe Acrobat software may enter their information directly into the electronic form.
Note: those without the means of opening or printing the online forms should contact FCSA at 877-553-4285 (or via e-mail at info@fcsa.biz). We will gladly send you an application form through alternate means.
Clients are reminded that once they have selected an application form, they are responsible for reading the form in its entirety. Special attention should be paid to the section entitled WHAT TO SUBMIT as it clearly describes the types of academic documents clients are require to include with their submissions.

If you want a Detailed Evaluation of your secondary school coursework in addition to a Detailed Evaluation of your post-secondary (university) coursework, an additional fee of $110 will be required. Please contact our office if you require this service.
